Finding the Right Level of Care

Home Care Packages are structured across four different levels to accommodate varying care requirements, ranging from low-level basic support to high-level complex care needs:

Level 1: For basic care needs.
Level 2: For low-level care needs.
Level 3: For intermediate care needs.
Level 4: For high-level, complex care needs.

How to Access a Home Care Package

Check Your Eligibility: Contact My Aged Care to arrange an assessment. An assessor will visit you at home to chat about your needs and determine your qualification level.
Receive Your Assessment Result: Once approved, you will receive a letter detailing your assigned package level and placing you in the national queue.
Choose Your Provider: When your package becomes available, select Mount Gollis Care & Services as your approved provider. We will help you navigate your budget, sign your agreement, and map out your care plan.
Begin Your Care: Your personalised support begins, with ongoing reviews to ensure your services continually match your goals and lifestyle.
